[5e OGL] Proficiency Bonus Not Adding

Whenever I make an ability check and click on the certain ability (i.e Performance) then it should add the proficiency with that roll (i.e 1d20 + prof). However, whenever I roll, the proficiency bonus is not being added. The only proficiency added is the proficiency given by the level of the class but not any extra proficients that the character and class received while leveling up. I was able to work this a few weeks back; however, last week I noticed that the proficiency was not being added to the ability check rolls automatically. Another odd thing for me is that if you're making Saving Throws, the proficiency is automatically added.  The picture below shows the prof. bonus added into the roll when I click from these. So there is no problem with this.  The picture below shows an inconsistent add of prof. bonus to the roll of abilities.  I noticed how Nature was added the correct prof. bonus but Perception and Performance did not have the bonus added.  It's not only those that do not have the prof. bonus added to their roll, it's the majority of the abilities. A few selected do include the prof. bonus. I'm a bit confused on what the problem might be.  I'm fairly new to Roll20 so I might have missed something completely. I do appreciate your patience and help :)

It would help immensely if you could say which character sheet this is, as there are multiple for 5e. This looks at first glance like a sheet issue rather than a Roll20 issue, so I'm moving this to Character Sheets for now.  If it turns out to be a Roll20 issue, this thread can be moved back here.  Good luck!

Oh oops!  I'm so sorry. I'm still new to Roll20 and is my first time posting something on the forums. Haha I feel embarrassed for my posting mistake.  This sheet is for a Bard (lvl 3) + Cleric (lvl 1). I don't see any tokens when I join the campaign. I will post what shows up after asking the DM to place the tokens on there.  As for the type of character sheet... I really don't know what type it is x_x so I took a screen shot.  It looks like this.  Also I would like to thank you for helping me >_< 
It's okay, no worries! I recognized the sheet to be the 5th Edition OGL by Roll20 Character Sheet (and tagged the thread accordingly). It looks like your Character doesn't have a character_name (the 'Character Name' field on the sheet appears to be blank). I'm wondering; did was that edited out for the purpose of the screenshot, or does your Character have no character_name? An alternative to using a selected Token is to:   + Add a new Ability (on the Attributes & Abilities Tab of your Character). Edit the Ability ('pencil' button). Copy and paste the following code into the Ability's body: @{perception_bonus} @{performance_bonus} Use the Ability (i.e. by pressing one of the 'd20' buttons before or after saving the Ability).
I completely did not realize I didn't put a character name on there haha xD  but just in case I did add the name and rolled my abilities again but it showed the same problem :(  When I added a new ability and put in what you told me to, I got a 0 and 2 in that order. What could this mean? x _ x

Your game must have been using another Character Sheet before switching over to the 5e OGL sheet. On the Attributes & Abilities Tab of the affected Character(s), you'll need to locate and delete all Attributes which have a name similar to: "PB" or "pb" " <skill> _bonus"; e.g. "deception_bonus" " <ability> _mod"; e.g. "strength_mod" To delete an Attribute, hover over it and press the x button that appears.
Oh my goodness, I think that's the case too!  I greatly apologize for not replying to this post. I was out of town >_<  but the first campaign, I used the character sheet for my first character. Thank you so much for your patience and your help!  You are truly awesome :)
