We've had quite a few issues with chat and voice where it literally just quits working. it's happened quite a few times now when some of the group was just talking while working on their chatacter sheets. restarting the browser doesn't fix it. then it finally works after disabling the chat+voice, re-enabling it and changing the output to the other then back again.
