Can you attach multiple tokens to one character sheet?

I am trying to determine the easiest way to play a Druid-type character with Wild Shape forms using the Pathfinder sheet--one of the quickest ways I thought of would be to create a Buff or two on the base sheet with the various stat, size, and armor class changes which could be toggled on and off (then simply have appropriate attacks already in place, like claws, bite etc). This would allow me to use the same character sheet for all forms. However the sticking point and the part we're not sure about is whether you can attach multiple different tokens to the same character sheet (so you can change the appearance of the character when in wild shape) so they would all be able to read the Buffs on the base sheet. Is there a way to do this?

Make a  rollable table with the 'faces' all being the different tokens. Then you can make a token from the table and set it up as you would a normal token, make that the default token for the sheet and use the right click 'choose side' option to change the appearance as needed.
