The ability to post images to chat is a roll20 feature. You can try inserting the image anywhere you like in your macro. The formatting/layout will be determined by the sheet's roll template. Larger images can mess up the layout. You can grab image url's within a browser by right-click "Copy image address". I did an image search with google for my example above and restricted the search to small clipart images.
