I believe this hasn't been done. Because any game can only have one character sheet assigned to it, in most cases this UC sheet would have to be somehow integrated in a otherwise "general" Pathfinder character sheet. If there is a special UC / Kingmaker sheet it is probably meant to be used in a separate campaign which contains only the kingdom building. Since I'm not firm with neither Kingmaker or the UC rules on that part I can only guess that this might not be useful on its own.
