Normal gameplay, APL 3 versus APL 9. we survived and won, but one of the dice rolls came up a little hinky. see if you guys can spot it as well. Yep, rolled a 5 on a d4.
Because the dice was set to explode it is more likely that they rolled a 4 and a 1 which was the result of that ‘explosion’. The second d4 only got a 1 so no explosion on that. Check out the wiki for more details.
Exploding dice as denoted by the ! or !! after the roll command re-roll on a maximum unless specified otherwise. The !! version compounds the rolls so in this case a 4 and a 1 to output a 5... check the link I gave, it should tell you all you need to know to understand how a 5 was rolled on a d4 :)
