Lycanthropy/Multiple "Forms" Question (Pathfinder)

On paper, when creating a lycanthrope, i normally use multiple first pages of the character sheet - one for each form - Base, Hybrid, Animal.  Are there character sheets available for Pathfinder system that allows this addition of "Main Tabs" or First Pages? Or am i stuck making three characters and potentially drag-and-dropping/deleting the seperate tokens each time a change occurs? Is there any other option people know about?

You could create a rollable table with the token images to give the character a 'multi-sided' token, then then create custom buffs in the sheet that apply any stats changes based on each form. I did something similar last year for a Shifter character, and had macros that used the scripts TokenMod and ChatSetAttr to apply the appropriate buffs and token changes for each form.
