Token defaults not being applied

Tokens don't seem to have defaults applied consistently regardless of browser used.

Can you give some repro steps, and some examples of what you mean? Have you read the help sections on linking tokens? Many folks mistakenly do the steps in the wrong order, which leads to default tokens that don't have the expected settings. Here is the wiki article on  Linking Tokens to Journals Here is a  video  which demoes the procedure
It's actually far simpler than that. I am just setting all of the bars to be visible in campaign settings and they come in the map with non of them visible.

Did you apply the settings in-game? Settings tab, near the bottom. Also note that changing and applying the settings will only apply to tokens on the tabletop or new ones you create. They will not affect any previously saved default tokens.
After you asked I gave it a shot but this didn't help either. I also don't really see why I'd be using the in game feature when there is already supposed to be a game default settings for the campaign as a whole. Is there any reason that the default game settings would be ignored and replaced with nothing being visible on tokens at all?

Setting the out of game settings affects new content at time of creation. Applying them in-game affects content already created, with one exception: previously saved default tokens. Roll20 doesn't touch your default tokens, probably as part of the same process which protects your home-brew alterations. Supposed you have brought a goblin into your game. You decided that instead of short swords, all your goblins use spears, so you alter your instance of the goblin character sheet so that every time you bring in a goblin, it comes in with a spear. If you drag in a goblin from the compendium, the first thing it does is check if you already have a goblin sheet in your game. If it exists, it places that one instead of the compendium copy. You wouldn't like it if Roll20 overwrote all your homebrew. This means that the default token settings (including bar visibility) will not be set on the compendium drag. They will be set to what you expressed to be your preference at the time you saved the default token (or very first dragged in a compendium creature that had custom token artwork). Try this: Once you have verified that the setting s are set and saved properly in your game settings (which it certainly sounds like you've done), drag in a creature that you know for a fact does not already exist in your game. It should be created with the settings you want. A more thorough test would be to delete a creature from your game and drag in a fresh compendium copy, but that could mess up any scenarios where you have already placed tokens on the map, since they will lose associations. If you are still having an issue after verifying this is not the problem, you'll likely need to fill out a  Help Center Request , after following the troubleshooting steps I'll paste below, and capturing the console log. Could you please carefully work through these steps of our guide to  Solving Technical Issues ? Step 1: Make sure to use the right browser Please check if these issues persist when using both Chrome  and  Firefox. Step 2: Ensure that there are no extensions/addons interfering with Roll20 Please disable  all  add-ons/extensions. Step 3: Clear your cache If none of the above succeeds, please thoroughly work through  Step 4 .  (Don't forget the complete Console Log!) Thank you!

Another, simpler possibility just occurred: Do the bars have both current and max values already filled in? They will never display if they do not have a max value.
I'll fill out a help center request, because this isn't even an issue of compendium creatures or creatures with pre-existing stats. It's simple tokens dragged in from library images having their bars hidden from players despite being marked as visible in default. I put in infomration and it's visible to me but remains invisible to the players regardless of what I do to the defaults. And yes of course I pull in a new image fresh for each test. This persists across all browsers.

Some screen shots might be helpful. I've not run across a bug like this before, so I still feel it is likely to be a procedural error.
Here are the screenshots I included in the bug report along with the logs. First is the default game settings. Second is a fresh token pulled in from my image library.

The logs don't show anything occurring when I pull out the token itself that would indicate errors or anything being blocked. It's just on a very basic level applying nothing to new tokens without compendium entries.

I don't think that's the issue. Given the game settings, any item dragged in from the art library (which is by default, a token), should have those options clicked on. When I do those steps, a library art item has them on, as expected. When McBeardly does the same, the options are not selected. This does not appear to have anything to do with linking to characters.
I am dragging them on to the board from the art library and this hasn't been an issue before. I tend to make custom npcs and monsters for all my games, so I'm not grabbing from a compendium, but changing PC side visibility for each token is a tedious additional work step.

Note that these are currently taking a distressingly long time to process. In addition, I would suggest creating a test game and see if the behavior duplicates there as well.
