Setting the out of game settings affects new content at time of creation. Applying them in-game affects content already created, with one exception: previously saved default tokens. Roll20 doesn't touch your default tokens, probably as part of the same process which protects your home-brew alterations. Supposed you have brought a goblin into your game. You decided that instead of short swords, all your goblins use spears, so you alter your instance of the goblin character sheet so that every time you bring in a goblin, it comes in with a spear. If you drag in a goblin from the compendium, the first thing it does is check if you already have a goblin sheet in your game. If it exists, it places that one instead of the compendium copy. You wouldn't like it if Roll20 overwrote all your homebrew. This means that the default token settings (including bar visibility) will not be set on the compendium drag. They will be set to what you expressed to be your preference at the time you saved the default token (or very first dragged in a compendium creature that had custom token artwork). Try this: Once you have verified that the setting s are set and saved properly in your game settings (which it certainly sounds like you've done), drag in a creature that you know for a fact does not already exist in your game. It should be created with the settings you want. A more thorough test would be to delete a creature from your game and drag in a fresh compendium copy, but that could mess up any scenarios where you have already placed tokens on the map, since they will lose associations.
