Fairly easy to reproduce. We have dynamic lighting turned on a map and the DM has enabled explorer mode and vision for each token. If a player picks up their token and moves it around without dropping it, dynamic lighting turns off, all vision is lost for that player. It's as if all the lighting sources were lost, but because explorer mode is on, we can see the map grayed out but no tokens. Reloading the browser tab fixes the problem and returns dynamic vision.
