I have a PC that got fleshcrafted two extra arms. They will be considered Secondary Slam attacks. He has a +15/+10 for his primary attack as a 10th level fighter. How do add the extra attacks? He is only getting one natural attack each arm at +10 and half his strength bonus (+2). How do I list these in the Weapon/ Attacks Menu on the character sheet?

Assuming the Pathfinder by Roll20 sheet: Add the attack normally Next, to accommodate the +10 instead of +15 put a -5 penalty in the attack line between "STR" and "vs AC" For half strength select STR (off-hand) for the damage.
