I want to minimise house rules for this game. One I can think of that is a nod to realism but shouldn't come up much unless the party has no healing at all: If a character is dying and stabilised on 0 hp, they are incapacitated on 0 hp for a week, or until they receive healing that restores at least 1 hp. In my game Polearm Master requires using weapon two-handed to get the bonus attack.
