Problem: Attack Rolls Not Showing, But Damage Does. Also A Secondary Issue With An Attack Macro Not Working In The Companion App

Last session, one of my players clicked on her weapon attack and it did this:

I tried it, too, and got similar results:

This is a +1 Spear that has been renamed Wolf's Fang.  The one-handed version of the attack works fine, but the two-handed version doesn't seem to be rolling the attack.  When I looked at the detail, both looked the same.

Additionally, the weapon has charges that can be expended on an attack, using this macro:

Expend charges for extra BITE! [[?{How Many Charges?|1}d6+?]]

I've deleted the weapon from the character sheet and dragged a new +1 Spear from the compendium and modified it as before.  It seems to work for me (both as GM and from a dummy account with access to this character sheet), but my player hasn't tried yet from the browser. 

HOWEVER.

The companion app seems not to like the charges macro, and it spits out this message:

Error evaluating roll starting with text, "[[" at position 435 near }} {{desc=Expend charges for extra BITE! caused by: Invalid roll: "1d6+?"

Oddly, when I tried it from the app with my dummy account, I got a slightly different error ("at position 429" rather than "at position 435") but I don't really know what any of it means:

I tried it with another character sheet with a similar weapon that uses the same macro to spend charges:

I'm hoping the big brains here can offer some solutions.  I haven't tried importing/transmogrifying an older version of her character sheet yet.  

Attack rolls portion of this is RESOLVED.  Just wanted to post a follow-up in case anyone else is having this issue.

Either the Character Sheet changed recently, or it's just a case of user error.  As it turns out, my player was clicking on the damage portion of the weapon in the attack section on their sheet, whereas I always click on the name.  

They said up until last week, doing this resulted in an attack roll and a damage roll and both displayed in chat.  Last week that changed and only the damage was rolling/displaying.  I didn't realize they weren't clicking the name of the weapon, adding to the confusion.  

The other piece is way less important (the app not liking the macro for charges), so I'm calling this one all done.
