Are you putting your inset on the map layer? Assuming the inset is opaque, then it sounds like you might be putting it on the GM layer.
Lots of possibilities here.
Is your PNG really actually a semi-Transparent one? Load it into a Photo Editor and export it as JPG, it will no longer have transparency. Then upload your new JPG version to Roll20.
Did you put it on the GM Layer? It will seem transparent if it's on GM layer but will turn Opaque if you move to Token layer, or move to Map Layer and "Move to Front".
Alternatively you can easily obscure a section of your Map, using the Roll20 drawing tools. This is the fast easy way to make an opaque shape of any size.
Alternatively again you can use Fog Of War, Roll20 feature, to opaquely obscure any part of your Map BUT it will be transparent for the GM-Only.

Definitely on the map layer.
First of all, when I looked at it as a player it was actually ok. So I can live with it for now That said, I might try converting it to JPG and see what happens.

Converting to jpg doesn't solve the problem. It's still transparent. I can probably live with it for now, as it turns out players see what I intended (but unfortunately I don't), but surely there must be a way of adding a patch to a map on map layer and the patch not being transparent to the GM.
Some things to try.
If those don't help, perhaps try unchecking the dynamic lighting settings on the map one at a time (Daylight Mode, Explorer Mode, Dynamic Lighting) to see if one of those is the culprit.
There are some situations in which a token on Explorer mode can appear translucent to the GM. Is Explorer Mode active on the page? If so, there are a couple of solutions:
1. Move graphic to the map layer
2. Give yourself as GM explicit control over the image (not just the default control the GM has over everything). You may need to give it sight as well, but I don't think that it is necessary.
