Flash Settings won't go away

I click "Deny" and "Close" and the dialog box disappears for a moment and then pops right back up.  I'm dual-boxing to test out roll20, and in both Chrome/Mac and Firefox/Windows it's doing the same thing.  We're going to be using Mumble instead of the built-in voice/video, so I want to just ignore Flash.  How do I make it go away permanently?
Nevermind.  As usual, once I finally post a question one more idea  occurs to me that fixes it.  I disabled voice and video from "My Settings", and although that didn't make the dialog go away I then switched the Avatar to "name only".  A workaround instead of a fix, but good enough.

If you refresh with the A/V disabled and Avatar not set to name only the dialog box will also go away.  - Gauss
