Linking character sheet fields to the token

I wanted to ask about something that seems like an obvious extension of the current system. As it stands the tokens are linked to the character sheet in the journal by the DM. Is there a way to link fields in the token (such as the bars i.e. hit points) to the character sheet? Meaning if I have a character who enters that he has 40 HP max and 34 Current, could the token reflect that?
The issue we are running into is the token is linked to the sheet, the group plays for a while and gains a level or two, and goes into a dungeon. Now since I have made a new page for the dungeon I pull the tokens from the journal for the characters, but their info reflects the old info from when it was first linked. Ok I can update the token, I get that. However, now the party has moved from the first floor to the second, so again a new page. The fighter has taken some damage and the group felt it would be ok to let him go on without healing up to full. When I pull the newly linked token it does not reflect that his HP are lower.
It gets really frustrating and seems to be an unneeded amount of bookkeeping when the bars of the token seem like they could and should link to the sheet and any change to one would be reflected on the other.
Please tell me I have just missed something.

The key points to note on the link GenKitty supplied are that if you change the information on the linked token you will need to edit the sheet to accept the new token... in other words make sure you have set everything as you want it before linking it to the sheet.

This doesn't stop you from changing things on the fly, it just means that should you wish to make permanent changes just make sure you remove the old linked token and add the new updated version =D

I would need many more fingers to count the number of times i've linked a token too early and been baffled when all my goblins start to lose hitpoints at once in some weird symbiotic hive mind way...!

i did this once, i was sooo confused when the monsters got hit with an AoE, and after i got done applying the -6 -6 -12 to the three monster, they all had 6 hp left, when they started at 30... i was like... ZOMFG u pwned my badies yer da monster! not them!
