I used to be able to click on a player's icon, and get a promoto to GM / Kick player option. Since Rugged Reroll update (maybe a coincidence) I cant seem to do that. Can someone give step by step instructions on how it's done, in case I'm missing something.
You should get the same options if you hover your mouse over the player's icon in the campaign details page. Are you not getting anything popping up? It should appear underneath the player's avatar icon.

I do see that in the campaign details page, but based on my testing, that doesn't kick a player while they are in a session, it only stops them from rejoining once they leave that game.

If you can't get the player to leave the campaign, I'd recommend this work-around (since I don't think there's a kick feature within a running campaign itself): Go to your campaign details page and click on the "Copy/Extend Campaign" feature. Duplicate the campaign and invite everyone to that one sans the annoying player. Once the player eventually leaves the campaign, you can kick them and go back to the original campaign or continue playing with the duplicated one. Whatever's easier.

All you need to do is kick the player and then have everyone refresh the game. The player will not be in the refreshed copy.

What happens if the GM refreshes, but some players do not?

Never mind, I was mistaken. It looks like my solution does not work (for some reason I thought it did).
