I have a player that has logged out and is always shown as logged in on the bottom left where names are listed. Any idea how to get rid of it without booting him from campaign?

It means that the player did not exit the game using the Exit Game button and instead exited the game by closing their web browser. The only way to get rid of it is to ask the player to re-join the game and exit properly.
