Dice rolling too much

A couple times in our most recent session we noticed that some d8's we roll are rolling above an 8 and a d20 rolled a 36.

Any more info?  Are these rolls from a character sheet, custom macro, directly in chat, the dice roller?  Were the roll results 3d dice or the actual roll result in chat?  Any screen shots? Sorry for the 3rd degree. ;-P

Are you using the exclamation mark ( e.g., /roll 1d20! )? That would make exploding dice that could roll those results.
