switched character to elk; broken on return

I'm using character sheets called "D&D 5E by Roll20" for my campaign named "Edge of Empire", and the player saw a floating button that said something like "do you want to wildshape (or change or polymorph or something) to an elk.  The player clicked yes, and we can't get his original sheet back in its original form.  For example, the proficiency is set at 2 and changing levels doesn't change it.

It sounds like someone tried to drag a creature out of the compendium or the journals and it tried to overwrite itself on the open character sheet. Never say yes. (I know, too late). The bad news is that although you could probably tweak it back, it wouldn't be trustworthy. There's no way of being sure that something critical was not overwritten. My advice would be to recreate the character, using what you can pull from the old one as a guide. If you switch it back to a PC from an NPC, you should have most of the details.

@keithcurtis: Thanks fore replying---that's exactly what happened.  @Dragon Dreamer: Yes, we are still having the issue.  For instance, the proficiency is stuck at 2 no matter what the level is.  The only option keithcurtis suggested is starting over from scratch.  We'll do that in a week or so when we have the next scheduled game if you can't fix this on your side.
