Editing D&D 5e Character Sheet to use Variant Option: Skills with Different Abilities

Instead of automatically assigning an ability to a skill, I was wondering how to go about editing the sheet to prompt a query to choose an attribute whenever a player rolls a skill. For example: A character is making a long swim to shore and is making a Constitution check to get there safely. It makes sense to be able to use the character's proficiency in Athletics. The player selects " Athletics " and a query pops up with a dropdown menu. The player selects " Constitution ." The character sheet then rolls using both values. Additionally, would it be possible to do the opposite, too? Select " Constitution ," and have a query asking for which skill to use (if any)?

The only way I know to do this with the official sheet is to create custom skills in the custom skills box. These can be set to "Query" at the time of rolling. The Shaped Sheet does this as a base feature.

The 5e Sheet by roll20 has a section to add custom abilities. In your case, you could make one, called "Swimming" and make the roll be Con, or call it "Athletics" and have it show you a query. It's not quite what you are asking for, but editing either of the 5e sheets is not an easy task, and this is a easy existing feature.
Thanks! These are both good workarounds. It's kind of a bummer that it isn't built into the OGL character sheet more easily, but at least a way does exist!
