Character Perception checks etc. do not have their proficiency bonus added to them

Below is an example of the error that is occurring and the character sheet that I am using. Any help is appreciated. Thank you.

What is the name of the character sheet you are using? Every sheet uses different attribute and ability names.

The character sheet is d&d 5e scrolls

Not sure then, I just created a game using the sheet and made them level 2 with a 14 dex and proficient in Acrobatics. It rolled with a +4 and the formula looks extremely different from what you are showing. Sorry, I just don't have any experience with that sheet. In fact I don't think I've ever seen anyone post about it before. If I could ask is there a particular reason you or your GM picked that sheet?

I believe the reason he choose this sheet is because of its relatively friendly user ui. There are a lot of inexperienced people playing the campaign and he wanted to make character creation as simple as possible
