How can i add discord chat and healt bar to my roll20 layout?

Hi there, i was watching a campaign on twitch and i see this beatiful layout: for this, i want to ask you: how can i add that beatiful discord pop up and healtbar near them? Thats not something you can do within roll20 natively, so we can only guess at what he's doing. Maybe he's running an extension that inserts itself into roll20.&nbsp;
