Healing Word Cast Level Addition

I am using the following template to cast Healing Word directed at a specific PC: &{template:npcaction} {{name=60 feet}} {{rname=Healing Word}} {{description=@{selected|token_name} heals @{target|Guy|token_name} for [[1d4 + 5[WIS]]]}}  I am hoping to add the option to cast at a higher level and to change the healing as such. Can anyone assist with this?
Change the [[1d4 + 5[WIS]]]] to [[?{What Spell Level?|1|2|3|4|5|6|7|8|9}d4 + 5[WIS]]] It will give you a drop down to choose the spell level when you activate the macro.
Thank you Dakota, but unfortunately simply doing this didn't work. It does still allow me to choose a target and now to choose a spell level, but it does not include the sentence that says "Tai heals Lara for 9" below the burgundy line below Healing Word 60 feet.
I imagine it would be best if I laid out the specifics here, apologies: I am using 5e and trying to build this as an ability Macro shown as a token action.
Did you remember to put the }} at the end of the string I gave you to close the description entry in the template?
