Turn Counter Initiative Number Increasing

I am sure we are doing something wrong, but not sure what.  When we add a custom label for a monster or creature that doesn't have a character attached to a token, OR customize the initiative number of a character it shows up properly the first turn (say a person rolls their initiative but forgets to click on their token first).  As we cycle through the initiative order, when that character comes up again, it gets 20 added to the number. For the most part this isn't an issue, so long as the turn counter includes all the characters/monsters in the encounter. But if we add an additional character to the order and try to sort by order, problems arise (the custom label gets pushed to the top of the order).  Has anyone else encountered this? Is there an easy solution?

For custom turns, the second box is the Round Calculation.  You put a number in there to adjust by each turn.  It can be positive (default) or negative.  You're probably adding an entry and then putting 20 in.  That will start you at 0 and add 20 each time the turn comes up.  What you need to do is leave that blank, then put the 20 in the Turn Order directly, after you've added the custom turn.
Thanks! I assumed it was something like that.
