[D&D 5e] Tokens, Character Sheets, and Wild Shape Question

So, in running D&D 5e, Druids and some other creatures have the ability to change their appearance and abilities in such a way as it would seem to require not just a Rollable Table that allowed for different Tokens, but having new character sheets attached to each change as well.  I suspect that that isn't really possible with even APIs, so I was curious how other people have worked this into their games?  One thing I could obviously do is just create a folder for the druid in question with a mini compendium of all the beasts and so forth that they are able to change into.  The only issue there is that you can't delete the old token if combat is going since that would also delete them from the turn order.  Appreciate any suggestions or solutions people are already using here.
