I try to spice things up a bit more than most GM's do. I don't strictly adhere to any set rules, and instead I use the rules as baselines for actions. I.e. You sneak up behind a sleeping guard. You don't have to roll to hit on that, I think it's kind of silly that you have a 40-60% chance to kill someone who's sleeping, so as long as what you're doing is reasonable, it'll be successful out of combat.
