I've seen this happen before in my games on occasion, but I noticed it happen in a game that streamed recently to thousands on twitch: <a href="https://youtu.be/juta33anVgQ?t=6947" rel="nofollow">https://youtu.be/juta33anVgQ?t=6947</a> (you'll notice at timestamp 1:56:07 that the die-roll does not match the number rolled in the chat log).
