PC Tokens stuck with weird generic image

I have submitted a bug report for this but wondering if some of you have encountered this before.  We have 3 PCs and for two of them, when they (or the GM) puts their token onto the map, the image in the token is this hooded spectre (which I presume is some generic placeholder but not sure) instead of the images the players have put in.  You can see in the screenshot below where my character Emerald has the correct image in the token, while the other two do not. Thanks for any help!

Could the images have been deleted from the art library (either the GM's or their own)? If they were, they will disappear from any character sheets, tokens, etc. once the image cache clears. Also yes, that is the "default" placeholder image that you're seeing.

Ah yes, that was the clue we needed.  Thanks! Our GM had deleted some of the art images and the players didn't notice they needed to update the token image (you have to scroll down a bit when you go to edit the Bio).   Fixed now.  Appreciate the quick response.
