Need some help for a custom sheet

Heya guys. Sorry at first, i have zero experience with programming or anything. So i wanted to start some Ten Candles One-Shots. Does anyone know this? I wanted to do a custom sheet for it (it doesnt need much..) 5 lines for the players to fill out.. If i got it, is there a possibility, that other players can see 1 of the 5 lines?

If you only need five entries, you could probably run without a character sheet by using the Attributes tab. This is how all games were run back before character sheets were introduced. If you have little to no programming experience, this would probably be the easiest way to go.
