I know this should be easy, and I've searched for the solution, but for the life of me I can't see how it's done - how do you remove duplicates of spells and items? Or spells and items you didn't want to add to a character sheet?

Click the lock below the section you’re dealing with then the trash canon the item in question then the lock again to turn off the function.
