Do you still have the layers menu on the toolbar and the page menu? I'm wondering if you're somehow getting logged in as a player.

This GitHub repository contains all of the Sheets currently available. Since this is not a Roll20 Official Sheet the best thing for you to do to get help would be to head to the Character Sheet forum to ask in our help forum specifically dedicated to the topic

The image shows an empty box where the fog of war icon should be in the tool bar.

Do you have any extensions installed?  If you switch to an incognito style view, do you still have the issue?
it work with inconito mode x) i have few extension
Hi Mag, You mention this working with incognito/private browsing, which may point to an interaction with an extension. Could you let us know which extensions you are using? Additionally, could you provide the name of the game you are seeing this? Does this also happen in a new game? We do recommend for using Roll20 disabling ALL extensions, but the flip side of that is folks who have a need for different styling for vision impairment or some other need, so if we can minimize difficulties there we certainly try.

Have you tried changing your account temporally to English? Does that make a difference? That specific icon in the menu is a character using one of the pictos fonts, if it helps anything. I tried out having my account set to french and login to a game with a custom character sheet, everything was normal(firefox). Turning it on and off didn't make a difference, and even when a (free) account in the game was a co-GM(like OP) it looked normal. I'd be surprised if this is connected to character sheet at all, but maybe not impossible.
