Is this possible? I have a player who is looking at playing a monster race warlock but having the evocation which allows disguise self continuously so they'd like to set up some tokens on the same sheet that they can swap over to depending on what they're disguising themselves out whilst carrying over sight, HP, AC and other stats the character sheet has.
