Is it possible to adjust the Initiative modifier for an NPC somehow, beyond the default +DEX modifier?

Aside from directly editing the initiative_bonus attribute, I don't think so? If it's something you do often, you could make a macro using the ChatSetAttr script.

I think that's precisely what I needed, I've never really had to edit attributes manually before but this should do what i want, for the occasional monster. Thanks.
