Major A\V Issues Last Night (11/8)

My regular group ran a game last night, and we came up against some major A\V issues in Roll20 interface. Anyone else?  Our group has been using Roll20 for a year now, with the same people, equipment, software, operating systems, browsers, the works. We rarely have problems outside the occasional reload. Last night seemed to be a nightmare. We noticed the icons indicating mute for video and audio had changed, but that was the only thing different with our setup that has been working for 5 different systems. When logging in, everyone was unable to hear any other member. We followed the A\V troubleshooting guide. After clearing the browser cache and double checking our connection to TokBox, we were still experiencing the problem. After tinkering with the settings, we found that setting 'receive from others' to video and audio, while leaving 'send to others' at audio only allowed us to talk. We normally do audio only setting on both. This fixed our issue temporarily, but several group members had the loss of audio problem re-occur sporadically throughout our 3 hour session. The only way to resolve it was to go through those same steps, changing 'receive' to audio only and then back again, after clearing the cache. We also tried moving to a different game session with a different DM, but the problem persists without doing those steps. No changes were made to this game from our last session, such as changing the flavor of WebRTC being used, unless the flavor that is used by default has somehow changed. We've never altered this in any session. If it's not broke, don't fix it, and all that.  Just curious if this was a one off thing that requires more digging on my part, or some sort of system wide issue. 
If it helps, I can confirm the exact same issue from a game room myself and a friend are in right now. We hear the sound just fine from any other source, and we can hear the off-focus 'pop' noise when someone uses the text chat. But we're getting infrequent messages about loosing a connection, and no audio from mics is being picked up (per your notes, esp. w.r.t. toggling the different A/V options under settings.).
Curious... do you have the LastPass extension installed and using a Chrome browser?

@Russell... Same with us. All other sounds, background music, and sound effects worked fine, it was directly related to player audio streams. Now that you say that, I do recall seeing a 'disconnected' message during our gameplay, directly above the player nametag window after resolving the initial problems. When this happened, the player that received the message would continue transmitting audio to the group and moving their token on the game board, but they could not hear anyone else until following those steps. @SkyCaptain... We do not have LastPass, but most of us are using Google Chrome. We did a test when the problem started with FireFox, but that had the same issues. I did check the current threads when the issue occurred and noticed that extensions, specifically LastPass, was causing problems. I did have the ChromeCast extension installed, but I disabled for testing purposes. Problem continued.
