I don't seem to be able to find where I can trigger a redraw of the page cover in the Page Toolbar, it just stays the same regardless of what the actual page contains? What exactly are the conditions that set the cover page?

Lifer4700, that is probably because you deleted an image from your Art Library without first deleting it from the map. Now it is a broken image. Alternately, you were using an image from a website and that image is no longer available.
