I had a quick look - that sheet appears to be broken. There's no easy fix - it hasn't been updated in a long time. Unless you want to grab a Pro subscription and re-code it, I think it's probably expired. It's not even adding PB to skills or saves when the checkbox is ticked. Whether or not it ever worked, I have no idea - it seems unlikely though. Someone would have had to change a heap of HTML attribute names, or the script block itself, to break it that much.
