Editable form fields in handouts

I would like to have the ability to have form fields in handouts. These would allow someone with permission to edit the form fields on the handout to change information in the fields without having to edit the entire handout. This would also allow a GM to give players the ability to edit the fields without editing the handout. Use cases would include calendars, ticking items on task checklists, recording notes, keeping track of the number of times an event or count of an item. A lot of these use cases can  be handled by a character sheet, but a handout looks better when this is going into a marketplace product.
